A 32 year old lady was suffering from recurrent urinary infection, fever and backpain. She was treated for tuberculosis of the urinary tract many years earlier and was on single kidney with kidney failure at the time of her presentation to us. She was evaluated and a long segment lower ureter stricture was identified in the single functioning side. The patient underwent correction of the same with keyhole technique and the patient recovered well. The kidney function normalized and on follow-up the kidney was saved. Only few such surgeries (laparoscopic Boari flap) have been performed in single kidneys with kidney failure. The case was presented in World Congress of Endourology 2010.
